- Non-white background — grey, beige, cream, or any patterned or textured surface does not meet the DONIDCR biometric standard
- Glasses present in the photo — even minimal thin-framed or rimless prescription glasses are not permitted under Nepal's biometric ID specification
- Incorrect face size in the frame — the face must occupy 70–80% of the frame height; a head that is too small or positioned too high or low is among the most common technical rejections
- Head tilt, turn, or obscured features — even a slight head angle or hair falling across the face will cause the photo to fail biometric verification checks
